New Patient Information

Please review the following:

  • Your appointment time is reserved specifically for you. Because of this, missed appointments or late cancellations are extremely detrimental to our day. As a result, we kindly request confirmation of all appointments, and at least 24 hours advance notice if you will not be able to make your appointment. Any appointment still unconfirmed by 24 hours of the appointment time may be cancelled in order to preserve the available time for other patients. Same day cancellations or no shows will incur a $50 charge.

    Repeated missed appointments or late cancellations may result in fees or dismissal as a patient.

  • Even with preauthorization, insurance will never guarantee payment. We verify eligibility for every patient, and we truly make every possible effort to ensure accuracy because we want you to have a great experience. However, it is ultimately the patient's responsibility to know their insurance benefits. Every insurance company is different, they will not guarantee coverage or eligibility, and can even refuse to provide specific dollar figures. If specifics are not provided by your insurance company, we will estimate based on what is standard for an average plan. Ultimately, your actual coverage may be different than this number and this should serve only as an estimate. You are responsible for any unpaid amount by your insurance company.

    Your dental benefits are based upon a contract made between you/your employer and an insurance company. If you have any questions regarding your dental benefits please contact your employer or insurance company directly.

  • Payment is due at time of treatment. Acceptable forms of payment include cash, Visa, Master Card, American Express, Discover, and assigned insurance benefits.

  • Our office is happy to offer an in-house membership plan for dental services at a discounted rate. Dental insurance often creates more of a hassle for patients. These hassles include but are not limited to: annual maximums, annual deductibles, frequency limitations, and downgrades for white fillings on back teeth. Sound familiar?

    Our membership plan is an annual investment of $400 and includes:

    - two healthy cleanings per year
    - two dental exams
    - all necessary x-rays
    - fluoride on recall visits
    - 25% discount on all dental treatment

    This loyalty plan requires no deductibles, has no annual maximum, and there is no frequency limit on any needed treatment. There is also a $25 discount for each family member on the plan, up to 4 members.

  • Upon scheduling, we will email you a link to fill out new patient paperwork electronically prior to your first appointment. We recommend completing all paperwork before arriving, however, if you’d wish to complete it the day of your appointment, please arrive 15 minutes prior to your appointed time.

  • Please call our office number at 205-857-3600 for any dental related emergencies. If during business hours, our team will do our best to accommodate you same day or within 24 hours. If you should encounter an after-hours emergency, please still call our office number, and an answering service will relay your message directly to Dr. Wiley. If able, she will arrange a time to see you in office for any necessary treatment.
